​ Disease Management:

    Disease management is a strategy of organizing care and services for a patient population across the          continuum. It is characterized by a population database, interdisciplinary and inter-agency collaboration, and     evidence-based clinical information. The effectiveness of a disease management program has been measured     by a combination of clinical, financial, and quality of life outcomes.

    Sally Mcc Group can assist your organization in implementing disease management as the foundation for a     new approach to performance improvement driven by your strategic planning process or as an improvement     goal for a high volume or problem prone diagnosis.

   An organization's QAPI System represents the translation of strategic, administrative and clinical      
   objectives into a group of organization-wide activities aimed at continuous and measurable improvement.  

   The cycle of improvement integrates  resident safety and satisfaction, quality of life and process redesign into        a dynamic approach to plan, design, measure, assess, improve and sustain performance based on a                      framework of inter-disciplinary communication and collaboration. 

   We can customize a project to fit your unique needs (see examples below) in a  transition to QAPI, including  
   current state analysis, training in improvement tools such as RCAs (root cause analysis) and FMEAs (failure    
   mode and effect analysis).

  have that  individual  on staff :
